Please read the following instructions carefully for next steps:
1. You should have received and email that contains some documents via DocuSign.
2. Complete the “SYMPTOM CHECKLIST” at the bottom of this page:
3.After completing those, please email me a copy of your photo ID, and let me know that the paperwork is done.
4. I will send you some dates for upcoming appointments and/or get you added to the waitlist.
5. Pre-payment is required to reserve your intake appointment. When we select a date, you will receive a text message from my payment system, IvyPay. Follow the prompts to set it up, and pre-pay for your appointment. Your appointment is not reserved until IvyPay is set up, and the appointment time slot may be reserved by a different client if this is not complete within 24-hours of you receiving the text.
6. Once your intake is secured, you will receive a link for our virtual appointment. All appointments are held virtually.
